C-MAID was created to help out-of-state patients and families explore whether Oregon’s Death with Dignity Act, often referred to as medical aid in dying, may be available to them. We help patients and families understand their options, prepare with dignity, and receive compassionate support in a calm, private, and carefully coordinated setting.

Begin with a Private Consultation

We begin with an affordable $299.99 private consultation. During this consultation, we take time to understand the patient’s wishes, medical situation, family concerns, timing, comfort needs, location preferences, hospice status, and available budget.

If the patient chooses to move forward with C-MAID services, the consultation fee will be applied toward the overall cost of the selected C-MAID service plan.

How the Process Works

Step 1: Complete the short consultation request below
The short form helps us understand who is reaching out, where the patient is located, whether hospice is already involved, how urgent the situation is, and how we may be able to help.

Step 2: Submit the $299.99 private consultation fee
After submitting the short consultation request, you will be directed to a secure Stripe payment page to complete the consultation fee.

Step 3: Complete the full private intake form
After payment is completed, you will be directed to the full C-MAID intake form. This form allows us to better understand the patient’s diagnosis, hospice status, level of care, medications, allergies, dietary needs, mobility needs, lodging preferences, family support needs, and comfort goals.

Step 4: Schedule your consultation
Once the full intake form is completed, you will be directed to schedule the private consultation. During the consultation, we will review the patient’s needs, answer questions, discuss timing, and explain possible next steps.

What C-MAID Helps Coordinate

C-MAID is not a one-size-fits-all program. Some patients need a clean, simple, peaceful space. Others may want a more private retreat-style environment with additional comforts, family accommodations, chef-prepared meals, linen service, legacy projects, music, art, remembrance planning, or other meaningful activities.

We work with a wide range of personal tastes, needs, and budgets to shape each stay around the patient’s values and vision, while keeping comfort, privacy, dignity, and pain-management coordination at the center of the mission from the very beginning.

Hospice and Medical Care

C-MAID is not a hospice provider. Rather, we work alongside hospice providers and, when appropriate, can help patients and families coordinate a transition into an Oregon hospice program so pain management, comfort care, and end-of-life support can be addressed during the patient’s stay.

As with customary hospice care, patients and families are generally not billed by C-MAID for hospice care during their stay. Hospice care remains provided, managed, and billed, if applicable, through the patient’s hospice provider, hospice benefit, Medicare, Medicaid, private insurance, or other applicable coverage.

C-MAID program fees are separate from hospice and medical-provider services. C-MAID fees are intended to cover selected non-medical support services, which may include lodging, coordination, support staff when requested or required, nutrition, comfort planning, legacy creation, artistic support, family support, and other selected add-on services during the patient’s stay.

Important Notice

C-MAID is not a medical clinic, medical facility, or medical provider. C-MAID does not diagnose, prescribe, determine eligibility, or make medical decisions. Instead, C-MAID helps patients and families understand the practical steps involved in Oregon’s medical aid-in-dying process, coordinate private lodging and support services when available, and, when appropriate, provide referrals to independent participating medical providers who can evaluate whether the patient meets Oregon’s medical aid-in-dying requirements.

Participation in Oregon’s medical aid-in-dying process depends on legal and medical eligibility, physician participation, required documentation, and the patient’s own voluntary decision. A consultation with C-MAID does not guarantee eligibility, approval, prescription, placement, or outcome.

What C-MAID provides is a compassionate starting point, a clear conversation, and a caring team prepared to help the patient and family understand the practical, emotional, comfort, lodging, family, and legacy planning needs that may come with the medical aid-in-dying process.
